A real-time tool for NVKC

Gaston Lab®

The Dutch Society for Clinical Chemistry (NVKC) wanted a real-time tool that supported the correct interpretation of laboratory test results, taking into account interactions between drugs.

The question:

Gaston Medical was asked to develop a real-time tool that supported the interpretation of laboratory test results by alerting to drug interactions.

This is how we worked together

  • The SMILE Working Group and Gaston Medical joined forces in a pilot project.
  • The basis for the pilot was the drug-test interactions included in the online database of the NVKC: 'What Interferes Where' (WIWA). The WIWA database can be consulted by laboratory specialists for daily validation of test results. A validation report was made of all possible interactions with a summary of the current literature and an advisory text for applicants.
  • Gaston Medical programmed more than 30 drug test algorithms based on the WIWA database, which filters lab and medication related information from the Laboratory Information System (LIS) and the Pharmacy Information System (AIS) or Hospital Information System (HIS). The algorithm combines and interprets this data and returns a possible interaction message to the LIS, which can be used directly by the laboratory specialist in validating test results.
  • All algorithms were validated with a fictitious patient file and then in practice with patient results. The accuracy of the possible interaction reports was validated with the medication and test results from the HIS and LIS by the laboratory specialist.
Installing and linking a new software application in a hospital proved to be no easy task. The experience of Gaston Medical proved to be very valuable here.

Technical integration

  • Gaston Lab has been successfully installed in various hospitals and linked to various LIS systems (e.g. MOLIS, GLIMS, Labtrain) and ZIS or AIS systems (HiX, Pharma).
  • With HL7, messages from the LIS, ZIS or AIS can be read and processed in real time.

The results

  • Partly due to the intensive collaboration with the SMILE working group, the Gaston Lab application can now report drug-test interactions quickly, effectively and in real-time.
  • Gaston Lab can process and analyze large amounts of data simultaneously and feed it back directly to the LIS or any other desired system.
  • The resulting Alerts are sent directly and automatically back to the LIS, so the laboratory specialist can take immediate action, for example, by discussing the signal with the pharmacist or treating physician.
  • By incorporating medication data into the interpretation of laboratory results, the 'Gaston Lab' application prevents incorrect conclusions from being drawn (incorrect diagnoses or delays in the correct diagnosis), which negatively affect the quality of care.
  • It is expected that Gaston Lab will be linked in several centres in 2019, so the value of automatic reporting on drug-test interactions can be further investigated.
